Novo Nordisk has re-launched its online diabetes support programme Cornerstones4Care, adding a new health coach function that it says will help patients manage their condition.
In addition to a personal health assessment the diabetes health coach provides patients with a tailored, 12-week behavioural change plan with a range of disease management tools.
These include trackers to help patients monitor their progress, reminders to help them stay on track, disease education resources such as fact sheets and e-books and prescription savings offers for eligible patients.
Christine Sakdalan, vice president of patient centric strategy and solutions at Novo Nordisk, said: “With Cornerstones4Care, we have always strived to provide custom support for diabetes patients. The enhanced website provides a new level of personalisation, along with comprehensive information that we hope patients and their care teams will use between office visits to achieve better diabetes care results.”
Novo’s re-launch of the programme will also add a new educational video series featuring healthcare professionals, chefs, sociologists, and YouTube personality Michael Stevens.
