Care in the Community (also called Community Care) is a policy adopted in the UK for care of physically and mentally disabled people in their homes as opposed to in an institution. This type of care is applied commonly to the elderly, people with learning, mental illness or physical disabilities who perhaps cannot travel alone to a hospital or centre.
